If you have Microsoft Word, click on the word icon on the left.   This will open the application form in Microsoft Word.  Save it on your computer as you would any other word document.   Complete the one page lease application form by clicking on each field and filling it in, then pressing the TAB key to move to the field.  When you are finished, save the completed form on your computer, PRINT the completed application out, SIGN the completed application, and FAX the application to Active Capital at 1-516-670-0105.

 If you do not have Microsoft Word, you may download the application form  and open it with Adobe  Acrobat Reader. Click on the Adobe PDF icon to the left, open the application, and print it. Then COMPLETE, SIGN, and FAX the application to:
